Avalon Case Management, Inc. was founded in 2000 by Kate Mood and Karen Moldovan, and has grown steadily each year since then. Kate and Karen are experienced registered nurses with business background, and have successfully managed several companies over the years. They are passionate about patient care, while striving to ensure the success and satisfaction of their clients and employees.
Avalon’s medical case managers are held to strict requirements:
- Must hold valid state nursing license
- Must have expertise and experience with case management of acute and catastrophic injuries
- Our staff holds and maintains all appropriate national certifications.
- Continuing education is required for all team members, with attendance at appropriate educational seminars and conferences to constantly maintain current knowledge in the field.
- Avalon uses the Advocate Case Management Software System, streamlining documentation and keeping personal health information safe and secure.

Avalon’s Senior Case Managers
Cynthia Hinkel, RN, CCM, CBIS
Cindy has been employed at Avalon since 2008. She has been working in the field of medical case management since January of 2002 with the primary emphasis in Worker’s Compensation and No-Fault Auto Insurance. Cindy has managed multiple cases telephonically and in the field. She maintains current certification of Certified Case Manager (CCM) and is a Certified Brain Injury Specialist (CBIS.) She has expertise in pediatric brain injury and multiple trauma resulting in catastrophic permanent disability.
Cindy primarily covers Central and Western Michigan and Northern Indiana. She is licensed as a registered nurse in Michigan and Indiana.
Patti Welborn, RN, CCM, CBIS
Patti has been with Avalon since 2010. Patti has been a case manager since 1999, and specializes in brain injury, spinal cord injury, orthopedic injury and chronic pain management. She has worked extensively with the pediatric population. She maintains current certification of Certified Case Manager (CCM). Patti covers the Kalamazoo, Grand Rapids and mid-Michigan areas.
